Application Process

Applicants must fill out the following online application form. The form includes basic information about the grantee. After submission we will contact you for additional information to establish compliance with our eligibility criteria, satisfy any legal or operational protocols, and run our funding process. Please accurately and cleary include a description of your need for funding on this form. After we contact the individual that filled out the form and evaluate the request, we will notify all requests of their accepted or declined application by phone and email.
